A CT angiogram showing the long-term result of a lower limb bypass graft in a now 71-year-old male patient. The image depicts a well-functioning above-knee popliteal artery to mid-anterior tibial artery bypass using autologous vein, tunneled deep.

The surgery was performed 8 years ago for critical limb ischemia. Today, the patient remains symptom-free, walks normally, and enjoys a fully active lifestyle. The graft is widely patent, with no evidence of stenosis or disease progression along its 32.87 cm course.
This case exemplifies the durability and efficacy of carefully planned and executed autogenous vein bypasses in select patients. Deep tunneling and optimal anastomotic technique have contributed to excellent long-term outcomes in this patient. No endovascular re-intervention has been necessary, and clinical follow-up continues to support the graft’s robustness.
In an era of expanding endovascular options, it’s important to remember that open surgical revascularization still holds a critical place — especially when durability, limb salvage, and symptom relief are the goals.
